Bixby Health and Safety provides additional support beyond CPR training. Clients can replace lost or damaged cards by requesting duplicate cards. Employers may request card verification for courses taught by Bixby Health and Safety.

Option #1 - By Mail
All items below are REQUIRED to process your
request:
- Enclose a note that includes as
much of the following information as possible. The more information
provided, the easier it is to find your course entry and reprint your AHA
card(s).
- Name on card
- Class type (BLS, Heartsaver,
etc.)
- Date of class
- Instructor name
- Your contact information (phone #
/ email) if additional information is required
- Enclose a self-addressed stamped
envelope to mail your AHA Provider card back to you. Use a standard letter
#10 envelope. The address you write on this envelope is where your card(s)
will be mailed. Postage is REQUIRED, meeting USPS First Class Postage
rate.
- Replacement fee - $15.00
for EACH card (e.g. BLS, Heatsaver, etc...) in the form of cash
or check (no credit cards accepted)
- Make check payable to Bixby Health
& Safety.
Mail the note, the self-addressed stamped envelope, and
replacement fee for the card(s) requested to:

Option #2 - Phone/Pick Up
When the student has an urgent need for replacement/duplicate
card(s), contact Bixby Health & Safety Training Site by phone.
